The GSS Supplemental Benefits Plan (also known as the GSS Drug and Dental Plan) is a supplemental plan that ensures coverage for costs outside of Ontario Health Insurance Program (OHIP).

This form should only be completed by students who have applied to the GSS to opt-in to the program, and who have received an email confirming that they are eligible and that their application has been accepted.
